ROLLing eX.tre.me is a 3D racing game based around street luge that involves riding a street luge board on your back down a paved road or course. The game uses a sitting arcade cabinet with a handle on each side and metal plates to rest the feet. Steering is done by moving the body left or right, causing the chair to tilt slightly. Players race down busy courses filled with traffic and obstacles while staying ahead of a large amount of opponents. Courses have a time limit with checkpoints that need to be reached in time to continue playing. The buttons on the handles can be used to hit opponents when passing them, swinging left or right. The feet pedals are used to perform sharp turns left or right.

Players need to reach a top spot to qualify and continue racing a next course. There are four characters to choose from: Buzz (New York), Daydra (Florida), Konkret (Germany), and Squat (California). Two cabinets can be linked up for multiplayer games with two players racing each other and other opponents simultaneously.
