Elevator Action: Death Parade is a lightgun game. Players take on the role of two agents, Zach A. Rainwater and Irina Isabella, who are called in to investigate what happened to classified data on P01 cells of the US military's Criminal Investigation Division, which has gone missing alongside previous CID personnel who have investigated it on undercover missions.

The game is the first in the Elevator Action series to abandon its original core gameplay in favor of the rail shooter genre. The major difference over other titles in the genre is the incorporation of physical elevator doors into the arcade cabinet, which open, close, and stay partially fixed in space in accordance with the on-screen action. Quick time events involving players forcibly closing the door with their buttons also occur during several points in the game.
